
The Florida Department of Heath in Jefferson County works with community partners to implement evidence-based interventions through the Heart Health Plus Program.
Activities include the Healthy Heart Ambassadors Blood Pressure Self-Monitoring Program, community health screenings, and process referrals to diabetes, tobacco cessation, and other health and social service programs.
The Blood Pressure Self-Monitoring classes are conducted by a trained Health Educator and Registered Dietitian. The program offers health education centered around nutrition, lifestyle changes, and blood pressure management.
Participants must enroll and complete the 12-week program. To enroll, participants must be clinically diagnosed with hypertension.
