Who can bring Children for Immunizations?
Children under 18 years of age need to be accompanied by a natural or adoptive parent, legal custodian, legal guardian, or person who possesses a power of attorney to provide medical consent for the minor. A stepparent, grandparent, an adult sibling of the minor, an aunt, or an uncle can bring the minor child in for immunizations, but they MUST have notarized written consent from a natural or adoptive parent, legal custodian, legal guardian, or person who possesses a power of attorney to provide medical consent for the minor.
Reminders for children’s immunization appointments:
- Submit immunization records to the health department PRIOR to your appointment. This includes all out-of-state and out-of-country immunizations. The clinic does not request immunization records on behalf of parents/legal guardians.
