Florida Department of Health in Jefferson County offers comprehensive childhood vaccines and limited adult vaccines.

What to bring to your visit:

  • Copy of all immunization records
  • Photo ID
  • Insurance Card

Child Immunizations

Review Vaccine Information Statements about each of these immunizations.

The list of childhood immunizations below are available at no cost:

  • Diphteria, Tetanus, Pertussis (DTaP)
  • Polio
  • Haemophilus Influenzae type b (Hib)
  • Hepatitis A
  • Hepatitis B
  • Measles, Mumps, Rubella (MMR)
  • Varicella (VZV)
  • Tetanus, Diptheria, Pertussis (TDaP)
  • Pneumococcal Conjugate (PCV)
  • Human Papillomavirus (HPV)
  • Meningococcal ACWY
  • DTaP+Polio
  • DTaP+Polio+Hib
  • Measles, Mumps, Rubella+Varicella (MMRV)
  • Who can bring children for immunizations?

Children under 18 years of age need to be accompanied by a natural or adoptive parent, legal custodian, legal guardian, or person who possesses a power of attorney to provide medical consent for the minor. A stepparent, grandparent, an adult sibling of the minor, an aunt, or an uncle can bring the minor child in for immunizations, but they MUST have notarized written consent from a natural or adoptive parent, legal custodian, legal guardian, or person who possesses a power of attorney to provide medical consent for the minor.

Reminders for children’s immunization appointments:

  • Submit immunization records to the health department PRIOR to your appointment. This includes all out-of-state and out-of-country immunizations.
  • The clinic does not request immunization records on behalf of parents/legal guardians.

Adult Immunizations

Review Vaccine Information Statements about each of these immunizations. 

The following immunizations are available. Please call for eligibility requirements:

  • Tetanus (TDaP)
  • Influenza (When Available)
  • Measles, mumps, and rubella (MMR)
  • Human papillomavirus (HPV)
  • Rabies-Post Exposure
  • Rabies-Pre-Exposure
