COVID-19 Vaccination Information

The Department of Health-Jefferson received our initial doses of COVID-19 vaccines today, December 28, 2020.

The Executive Order 20-315 directs us to focus on the following populations during this initial phase:

  • Long-term care facility residents and staff;
  • Persons 65 years of age and older; and
  • Health care personnel with direct patient contact.  

The Executive Order also allows hospitals to vaccinate persons who they deem to be extremely vulnerable to COVID-19. 

CVS and Walgreens have contracted with the state to provide vaccinations to long-term care facility residents and staff.

DOH Jefferson is coordinating with local private health care providers to begin the administration of the vaccine to their front-line health care workers.

We have temporarily stopped taking vaccine consent forms while we provide vaccinations to the 1,100+ individuals who have submitted forms. We ask for your patience at this time. We will announce when we are ready to accept forms again.

When we are accepting forms again, Florida residents 65 years of age and older should email the completed and signed Moderna Vaccine Screening and Consent Form to to be added to our vaccination schedule. The completed form can also be delivered to DOH Jefferson. Once received and reviewed by our nursing staff, you will be contacted to schedule your vaccine administration appointment or directed to participate in our mass vaccination clinic anticipated the first week of January. 

As additional vaccine is made available, the vaccines will be provided to the general population in a coordinated, orderly and efficient manner to help slow or stop the spread of COVID-19 within the community. Look for more information on vaccination clinics in the future.
